Comprehensive Initial Nutrition Assessment | Duration: 90 - 120 minutes | $220
Required for all new clients | Individuals under the age of 18 years must be accompanied by a parent/legal guardian

Follow-Up Appointments | Duration: 55 - 60 minutes | $120
Follow-up frequency varies based on individualized needs/treatment goals | Most clients find two to four sessions per month to be most beneficial

Payment & Insurance Information
EMPOWERD Nutrition Counseling is an out-of-network provider (i.e, we do not accept insurance nor file insurance claims on behalf of clients). Full payment (via cash, check, or credit card [HSA/FSA accounts included]) for services rendered is due at the time of the appointment. Depending on your individual insurance plan and benefits, our sessions may be eligible for reimbursement from your insurance provider. EMPOWERD Nutrition Counseling offers monthly superbill documentation (i.e., specialized clinical receipts) that clients may choose to submit to their insurer for reimbursement purposes. Please note, a referral from a medical or licensed mental health provider (specifying relevant medical/mental health diagnoses) is needed for a superbill to be generated with diagnostic coding.

EMPOWERD Nutrition Counseling offers specialized medical nutrition therapy and nutrition counseling to individuals located in Ohio, Massachusetts, and Texas.
Cancellation Policy:
All appointment cancellations and changes must be completed at least 24 hours in advance. Missed sessions and late appointment cancellations/adjustments (i.e., changes made less than a full 24 hours prior to the scheduled start time of the appointment) will result in a late cancellation/missed appointment fee of $120.
